Data: details & interpretation
Excerpt from the LINACS publication (Sundahl et al., Radiother Oncol 2026):

“For anti-vascular endothelial growth factor (VEGF) mAb, fifteen studies[1-15] (six prospective and nine retrospective) included 411 patients treated with this drug within one month of SRT (wmFUP: 26.4mo). Most patients (≥394; 95.9%) received intracranial SRT, and ≥353 received bevacizumab. HgOAEs were reported in 3.3% (10/301), all of whom received intracranial SRT. HgRTAEs occurred in 3.9% (4/102), of whom at least 85 received intracranial SRT.”

Radiobiological rationale for RT–drug interaction
Radiosensitizing via vascular normalization and endothelial VEGFR2/AKT/DNA-PKcs blockade. Endothelial injury is not tumor-restricted, so the mechanism provides a clear rationale for increased vascular normal-tissue side-effects—bowel injury, ischemic colitis, GI perforation/fistula, and impaired healing—particularly where the field includes bowel or previously irradiated tissue, plus a theoretical worsening of radiation lung fibrosis (Hamdeh et al., Alimentary Pharmacology & Therapeutics, 2021, citing Hapani et al.; Makita et al., International Journal of Clinical Oncology, 2026).
The data above refer to patients treated with VEGF mAb as the sole drug class, excluding any concurrent use of drugs from other groups. Combination therapy data are available in the section below.
